Sentinel lymph node biopsy

چکیده

Editor—Kell and Kerin wrote about sentinel lymph node biopsy in breast cancer and melanoma. This is a staging procedure with a well established role in breast cancer, reducing the need for dissection of the axillary node. The indications and advantages for sentinel lymph node biopsy in melanoma are not confirmed, and it is certainly not the established treatment implied in the editorial. Sentinel node status in melanoma is indeed the best staging and prognostic indicator: patients with negative results have a better prognosis then those with positive results. However, there is no evidence from randomised clinical trials that completion lymphadenectomy (sometimes known as selective lymphadenectomy) in patients with positive sentinel node biopsies offers any survival advantage. Neither is there any adjuvant therapy that can influence the natural history and benefit patients who are sentinel node positive. Concern has recently been raised about the possible increased incidence of local and in-transit recurrence in patients with positive sentinel nodes who undergo completion lymphadenectomy. This iatrogenic complication carries an ominous prognosis and is probably explained by lymphatic obstruction and entrapment of melanoma cells in transit to the regional nodes. The final decision for or against sentinel lymph node biopsy in melanoma must await the result of a multicentre selective lymphadenectomy trial, which will not be published before 2006. Until then, patients deserve to be informed that sentinel lymph node biopsy in melanoma is an investigational procedure with an unknown outcome; the significance of a possibly increased incidence of local or in-transit recurrence should be included when obtaining informed consent. A degree of editorial balance in the literature should be encouraged, rather than the present unbridled enthusiasm for an attractive but unproved concept.

Sentinel lymph node biopsy correctly predicts regional lymph node recurrence in trunk malignant melanoma with multiple drainage basins

We report a young male with an initial excisional biopsy report of melanoma of the lower back, referred to our hospital for complete excision and sentinel lymph node (SLN) biopsy.  Four peritumoral intradermal Tc-99m phytate injection was performed and SLNs were detected in both axillary and right inguinal regions. On the biopsy only the right axillary SLN was metastatic leading to right axilla...

The need for skin pen marking for sentinel lymph node biopsy: A comparative study

Introduction: There is a consensus in the literature that sentinel lymph node biopsy is the standard procedure for axillary staging in early stage (I and II) breast cancer patients. Usually during lymphoscintigraphy, the location of the sentinel lymph node is marked on the skin by an indelible ink. In this study we evaluated this issue in our patients. Methods: 40 ...

Feasibility of sentinel lymph node mapping in renal cell carcinoma using intraoperative radiotracer injection

Introduction: The applicability of sentinel lymph node biopsy for early detection of metastasis in patients with renal cell carcinoma (RCC) is still in the validation phase and under investigation, which might be due to the unpredictability of the lymphatic pattern in RCC. In this study, we aimed to evaluate the feasibility and accuracy of sentinel node biopsy in patients with ...

Sentinel lymph node in cervical cancer

Background: Cervical cancer is the second most common type of cancer among women. Effective screening programs can help cancer detection in early phases and reduce death. Metastasis to lymph nodes is one of the most prognostic factors in patients who underwent surgery. Also, a positive result from pathology report alert oncologist as a cause of death. Sentinel lymph node biopsy has been widely ...

Sentinel Node Mapping in Non-small Cell Lung Cancer Using an Intraoperative Radiotracer Technique

Objective(s): Lymph node metastases are the most significant prognostic factor in localized non-small cell lung cancer (NSCLC). Identification of the first nodal drainage site (sentinel node) may improve detection of metastatic nodes. Extended surgeries, such as lobectomy or pneumonectomy with lymph node dissection, are among the therapeutic options of higher acceptab...
